You can kill around 20% of a level's enemies before dipping into High Chaos, and even then if you do Low Chaos in later levels you can go wild in the earlier ones. And then there's the fact that the game is built with replayability in mind due to powers, playstyles, gadgets, levels design, and plot choices. It's literally a non-issue.

You say that, but I do want to point out that the game actually was planned to give you a choice when it came to the White Phosphorus attack. You were supposed to be able to refuse to use it and fight through on foot, but they had ran out of time to implement it.
I think Spec Ops is a good game, possibly even a great one, but I do think the meta narrative in it cheapens the story. There's just a bit too much in the game that plays into the "you're a dick because you kept playing" angle to be as satisfying as it could've been. If they had just kept that stuff out and focused more on the concept of duty vs morality, or why Walker's savior complex is destroying his own mind for justification of his actions, it would be a far better experience in retrospect.

There are no good outcomes to that quest. You might argue that the Diamondbacks are the least disruptive, but it's still shitty. If anything, the disorganized raiders that fill up the canyon after the nuke is disarmed would be easier for the rangers to clear out at some post-game point than either the Militia or the radioactive fallout.
