So how can you tell?

Are you trying to imply homosexuality is just a meme?

There are three ways to introduce a gay character in your video game (or in fact, in any piece of media, but for vidya you have a lot more freedom in how you flesh them out).

The first way is the background gay. The character just happens to be a disgusting homosexual. You can see it by the fact that there is another man in his house, by a few selected lines with who is hinted to be his significant other, or by the fact that he politely states he's not really interested in women during an NPC conversation.
Said character is usually just a 3rd-importance character which your player shouldn't be too worried about.
Usually those kinds of characters are clever easter eggs, but without proper development they can feel artificial. This is the main pitfall.

The second way is the audio log gay. The life and tribulations of your lad is narrated through audio logs or letters scattered across the game world, in which he subtly references his lover (among, of course, other things helpful to world-building). For emotional points, you can have the player deliver those logs to the lover in question.
A good example of that is the letters exchanged between the two rangers in Firewatch, a bad examples is in Borderlands 2 (a scientist being tortured ; he is never mentioned again).
The main pitfall here is how to subtly say your character is gay without being too gauche.

The third way is just to have a gay main character who is obviously gay, but also very important for your game and for the story. His love life is secondary to his role in the story, and your player should first develop some affection for the character (as a friend, or as a bro) before it is introduced that they're a fag.
Basically, the opposite of the first option : a gay main character with a lot of personality, including the fact that he likes men.

Make them actual good characters. It's really not that hard, it's just that Western writers for some reason CANNOT write a good homosexual character. You can just tell they started writing with "okay we need a gay character" or at the end of a character's creation tacked on "oh they're gay by the way!".

Japan is better at it, just like they're better at writing for games in general anyway.

Did you even read what I said? You can tell it's forced because the game only seems to care about a character's sexuality if they're gay, and doesn't give a shit otherwise. The developers want to make sure you know every gay character is gay so they shove it in your face.

There's no need to create gay characters (or of any other sexuality). Just make them cute and let the fans do whatever they want with them.
Your image is a great example of what I'm saying. Since ZUN lets fans take a lot of liberties with Touhou you can find a lot of hetero, lesbian, futa and even yaoi works of the characters, and since the official character descriptions are very vague there isn't a single headcanon established.

One factor that matters is the setting, is it a place where LGBT characters are accepted or a place where they have to keep their identity in the shadows? In a setting like the former it makes sense for an LGBT character to have public relationships and it shouldn't be a problem that they have relationships or people they care about that they mention or show in the game or it's outside materials. But in a setting like the latter, subtlety is required, since no LGBT character would be out and about flanderizing their relationship and identity to every stranger they come across. That's why Andromeda's trans character was so awful: He/she/whatever told the MC they were trans and deadnamed herself in a society which supposedly didn't accept that, but the writers had to mention it for brownie points. Same applies to gay characters in that sense.
