Just make Duke more of an actual grizzled action hero rather than a 90s caricature. What made Duke work as the hero was that aside from a few little gags, the world around him was really grounded and dark. Forever made every single character in that game as jokey as he was and so a lot of stuff didn't land well. Duke 5 would need to perfectly replicate the tone of 3D or the 2001 version of Forever to actually work. I don't trust Gearbox to do that though

Setting aside that people would force themselves to like Duke more to adhere to their sense of political incorrectness, Duke is too much of a relic for this post-ironic society for anyone to seriously think he's entertaining or badass for reasons other than nostalgia, because everyone else would just rap on him for his lack of self-awareness since genuine macho fantasies will nowadays just get penned down as 'cringe'. No matter how you put it, Duke is a hasbeen, both in terms of personality and relevance. I only need to say BAZINGA and everyone will know what I'm talking about.
The only way Duke as a character could really work nowadays without coming off as a soulless rehash of the original is by acknowledging his social status as a hasbeen. One where Duke got overweight, had to move to a trailer with his twin wives (one which left him for some guy named Randy and the other is a bitch) after Forever sold like shit and left him with crippling PTSD when he hears the word 'forever'. He's trying to relive his glory days like all his other FPS buddies did, but none of his old fans want to shake hands with him because nobody wants to shake hands with a man who grabs his turds. His Ego bar is basically non-existent, the only thing protecting him from physical attacks is his body fat which needs to be refilled with junk food and alcohol. He says his one-liners more out of habit, but nobody really thinks they're cool anymore. His Jetpack can't last more than three seconds because of his weight. And nowadays there's all these fancy black female action heroes everyone looks up to instead, and all the babes to be rescued are not really babes.
However, as the game goes on and the more he overcomes his own depression, he steadily recovers his self-confidence as his ego bar grows larger, and by the end when Duke is back, he will have to find his place in this new world. Maybe he'll set out to find the Planet of the Babes, and return to Earth when he's needed again.

Duke was a parody/rip off of 90s and 80s popular culture down to his oneliners and references in the games, and the macho cliches he embodied were outdated and thus ridiculously funny even when Duke games were in their prime. If the character was brought back as is, lots of comedy about him would just fly over zoomer's heads and I don't like idea of updating Duke into parodying 2000s popular culture.
That's also the main reason why despite it's solid gameplay and impact on the genre, young gamers aren't really that much into Duke, even old Doom and Quake games have occasional surges in interest of zoomers. Duke Nukem isn't really timeless character, he and charm of his games were very tightly tied to cultural climate of their era.

PCism is only half the problem. Duke was funny because Duke levels were more interactive (hookers or billiard tables) and because he spouted movie quotes. For the 90s this was unique and really funny, but it stopped being funny when everyone got online and did it to death. What movies would Duke even reference now? Either he's using dated one-liners that only men in their 40s will get or he'll be referencing garbage movies nobody saw over the past two decades.
That's the problem. He's satire in a world that has become such a joke that it can't easily be satirized. Duke made fucking Monica Lewinsky jokes, you can guess how well a Stormy Daniels joke would go down with /pol/ or discord trannies (answer: poorly).

I could see it working if they came up with a new enemy. Generic aliens don't work anymore, they're boring and the sex jokes derived from them fall flat. Going way back, one of the big reasons WW2 shooters ascended in the early 00s was because the enemies were different and seemed threatening. This is especially notable with Bioshock, which isn't a WW2 shooter but whose NPCs had a lot of character in them. Same for Postal 2, although Postal 2's mechanics are so different it at least let the player shoot cops which 3D Realms never had the guts to do (hence pig cops).
Cast Duke as a damaged Benoit-like character in a broken world, fighting off junkies and actual cops while hitting up the few good looking women tucked away in hidden places. Set it up like the Terminator, where Duke has to go to LA for something but has to fight his way into heroin factory or something and stop the US from being mind controlled. It'd just become a copy of Bioshock though.

What was politically incorrect about Duke Nukem 3D? The "aliens are taking are women" plot, a few strippers and that "Time for some R and R" audio clip at the end? None of those are really important to what the game is. People have conflated Duke Nukem with womanizing when honestly you could take all of those elements out of the game and Duke 3D would still hold the same position as an important FPS game. It's about fun weapons, fast gameplay, and interactive levels. There are plenty of ways for Duke to be a big dumb action hero with wisecracks that don't involve strippers and even good writing could allow for some of those elements to stay in. There's nothing politically incorrect about a seedy red-light district being seedy.

But I do think they'd fuck it up anyway if they tried again. It also has the problem of the audience. A lot of Duke fans have given up, or died, or don't play video games anymore, and I certainly don't want Duke to be "modernized" for current audiences.
I would love to see the Duke return and do well, but I just don't think it's very likely.
