It's an indie game so you can only pick two

You should have known it would be short the second you saw it has nice pixel art and good gameplay.

Even with replaying it 2 times I only got about 9 hours into it. Shame though, game is pretty fun. I hope the future DLC will add a lot of content to the game, or at least conclude the ending cliffhanger in a neat way.

Does the game change for the second playthrough? or just some flavor dialogue?

Also is the little girl real or what, she's way too pure for this game.

You can do dialogue options you didn't do the first time, which gives you different conversations and scenes. Most of those are only small and don't impact the game much. But there is one significant one, you can fight a secret boss if you pull it off.

Every time you speak with your psychiatrist, go against his wishes and choose text options that will anger him. On assassination assignments, do everything the psychiatrists told you not to, like talking to the DJ and killing every cop in the prison stage. If done right, you will get an extra secret boss near the end of the game, which is honestly the most difficult one, with one of the best soundtrack in the entire game.

But Hollow Knight looks great, has great gameplay, and is like 40-50 hours long. Whats up with that?

For the Psychiatrist, always interrupt him, or pick dialogue options that question or avoid answering him?

Don't know if the girl is real canonically, but in my theory she is, including the masked men. Since they can interact with the real world (killing the cops, cooking a meal for Zero), you could take that they arel in fact real out of that. Either that, or Zero does those things by himself while he is having hallucinations, but that's unlikely, since I doubt he somehow kidnapped the girl himself at the end of the game.

Always interrupt him, except in the case of the session after the DJ. For that one you need to refuse the Chronos. Also, in the first mission you need to repeatedly hang up on him when he calls you.

Interrupt him, choose the red options always. In the session before the Prison stage, refuse to take the medicine. You also need to anger the receptionist at the hotel.
