Light doesn't escape the event horizon. Around the event horizon, light warps. That's why you see a light disk around a black dot.
There's a lot of guessing involved because it has been made using an array of telescopes where a lot of information is missing because the array doesn't have enough telescopes (even though the number has been virtually extended using Earth's rotation). The most accurate way to define it is as a very likely prediction based on real data.
And no, the prediction hasn't been made based on assumptions about what we believe a black hole looks like. They have trained the model using real pictures of both astral and terrestrial objects scenes.
