Isshin phase 2 & 3 are easier than Isshin phase 1.
Phase 2 & 3 are much more predictable than 1, and Isshin is also slower in phase 2 & 3.

Hit his vitality instead of his posture if you can't consistently take out his posture.
He is fucking LITERALLY one of the cleanest fights, you can counter everything he has, even his gay ass lightning bolt.

Not him but I also had a much easier time with phase 2 and 3. Spear just meant a lot of slow attacks and Mikiri counters and I was way better at doing those than deflecting his fast sword hits.
Phase 1 is 100% predictable when you stay on the offensive
If you resurrect 3 times by using a clothed Buddha, he keeps counting the Iron code, not sure if it goes past 3 though. Pretty cool.

I didn't grind, I just went ng++++ because bosses give you the most xp, I only had to get as far as senpou temple and the monkeys.
Have you even tried to learn his moves or do you just try the same shit every time like a retard?

This is the entirety of phase 2 if you just keep your distance. In phase 1 your influence on Isshin is nowhere near that high and you can't determine his moves like that by simply staying a certain distance like you can in phase 2.
And phase 3 is practically like phase 2, except that he will now occasionally do one of two thunder attacks, that you can send back to him to punish him. Or easily evade.
Upon being hit in phase 2 & 3, be it on block or full on hit, Isshin's possible retaliations are both slower and create more distance between you. It's not nearly as aggressive.
Phase 1 is less predictable.
Phase 1 is faster.
In phase 1 he will do several different kinds of combos with different speeds and timing when retaliating.
Phase 1 can easily mess you up by doing something unexpected. But phase 2 & 3 are basically smooth sailing all the way to victory.
REMINDER
You can parry every single one of Isshin's attacks, including the leaps, the wind attacks, and the gunshots.
The ONLY exceptions are the perilous sweeps (one of which he rarely uses, and mostly only at the end of attack combos, and another which he only does if you interrupt his iaido charge-up) and the lightning (which you can reverse and send back at him, which makes the last phase even easier than the previous one).
The only excuse for not being able to beat him is being bad and unable to learn a simple bossfight.

This boss was a mistake, it's not even hard, it's just an unfun slog. I beat LB, horse guy and owl on the first try, most with the bell on. It took me 4 fucking hours to get through the sword saint fight and it ruined the game for me.
I hope they make another game with sekiro's combat that has any enemy or area variety at all. Everything except fountainhead was the same boring shit over and over again.
This is the first from game I regret spending the time I spent getting good.
Don't have to reposition, or risk doing a shitty dodge (parry frames I feel are far more forgiving than dodge iframes). Parrying the melee-range wind attacks (like the charged circular sweep one) also deal posture damage to him like parrying any other melee attack would.
It's also just cooler than dodging.

Dodging can leave you vulnerable because of the start and end of the step dodge, but when running you've got full control of your movement and it's completely safe.
People stand still way too much in Sekiro, or just slowly walk/jog to the sides. It's not a Souls game, you can actually move in this game.
I wouldn't be surprised if you could posture break with deflecting them. You can on owl by deflecting his shurikens, it even teleports him in front of you for the death blow.
wind attack is easy as fuck to parry and if you're close enough for the actual weapon to hit it does a good chunk of posture damage.
This boss is not hard, he is testing everything you learned trough out the game.
Honestly I wanted to write some tips for you, but after that I realized that is useless.
You have gotten this far, unless you cheesed trough the entire game you should be able to defeat him.
I have beaten the game 4 times already but this is actually true
who else likes to just perfect parry Ishiin 1's sheathed shining attack, it makes this phase incredibly short

and it seems to trigger his unblockable stab so you also get rewarded in some way
I just beat him. In his 2nd phase just bait his leap attack for some free hits on his back and chip away at his vitality. The 3rd phase is easier because you can deflect his lightning attacks.
You either go poke strategy and just keep yourself far as fuck, but that's boring as shit mind you, or go ultraclose combat. Far easier to get some damage in and avoid his spear if you go into his guard. He forces you to stay midway, in complete range of the spear and that's where he fucks you hard.
Phase three is easy if you just catch and reverse a couple of lightnings.
