Yeah, the main difference is in the apparent effort by the devs. In ash lake if you look up and realize there's no way you scaled that far down, even though your immersion is broken you can say, "Well fair enough, I wouldn't want to spend 3 hours in a tree." and move on. With Iron Keep the only explanation you can think of after immersion is broken is that the devs fucked up/were lazy. What's worse is they just needed one or two items with descriptions like, "Fell from Iron Keep" or a lava waterfall somewhere and you would have the same thought of, "Fair enough, I don't want to sit in an elevator for 3 hours to get to to lava island".
