I mean, they probably have and do. Getting into and out of the Grand Line is only an issue for fledgling pirate wannabes, not anyone of merit. I imagine there are several stations in the New World where pirates enter and exit at their leisure to do business with the outside Blues.

Who's to say it's never happened? Not important anyway since there's not much of worth in the calm belt. Trying to take a shortcut to get to the end of the GL would be pointless since the way to get to raftel requires interpreting poneglyphs anyway.
You need to be able to read the red poneglyphs to be able to find raftel where one piece is. Everyone who can read it was killed except for Robin, and she's on that journey right now.

Probably the biggest one I can think of along with Crocodile somehow losing to Luffy despite being strong enough to take potshots at Whitebeard's crew later. Pre-Sabaody power levels were a little weird, Oda got his act together once haki started being explored in greater detail. He probably didn't expect to be writing the greatest selling manga in history at the time and was probably living in fear of being cancelled like most new mangaka, so he had to spice up the plot and pace near the beginning.
