SS is the entire realm. In the seireitei only shinigami live, the rest live in the rukongai districts

Shinigami have a high occupational hazard, also its more rare for a soul to have the capability to become a Shinigami.

Its implied in Burn the Witch that other Soul Societies exist, not just the East Branch. Rukongai is also much larger than what you're thinking of; most of what we see is right outside of Seireitei which itself is about the size of a city.
