Ive worked in Medical Insurance, shit is fucked. The billing system is intentionally made into a technical maze to frustrate both providers and customers to just giving up their claims. Even the customary and Reasonable amounts dictated for Non Participating Providers is not reasonable. Id understand why Americans would rather be taken by taxi rather than an ambulance to a hospital.
