The Avengers initiative and all the problems they caused made caused him to reprioritize his issues with The Hulk. He is visibly annoyed in Civil War that The Avengers have lost track of both him and Thor though, comparing it to the US Army just losing track of two nuclear warheads in terms of accountability.

Real Answer; Since Disney can't make money off another Hulk solo film thank to Universal Studios owning the distribution rights they don't care to acknowledge ANY Hulk material at all. That's why he's such a joke side character

Presumably she got sick of sitting around waiting for Bruce who disappears for years at a time and decided to get on with her life.

Ross explains this in Civil War. He had a heart attack and was forced to learn how to dial the rage down from an 11 to like an 8.5

Also his attention kinda shifted from the Hulk in particular to keeping the Avengers in general under control.
