Can't he work in a back 3? I'm thinking that he could do some damage as something of an extra midfielder who is never marked... But even on attack he stays back or bumps with other midfielders...
You've tried that and seen how it doesn't work. If you want the guy to participate in attacking moves, play him at DM or further forward.
Finishing and composure also matter, then again even with 20 in every stat sometimes you get some retarded misses here and there

composure/finishing are more important tbqh. have a cm on 100% conversion rate with 9 penalty taking and 15 composure.

Penalties are just luck in FM
The player's personality and mood also matter. If you're Hartmanning players without an elite character they can go to penalty kicks low on confidence.

I play as long as I don't get bored. Depends on how the save goes but usually between 8 and 13-14 seasons.
