Oddly enough a pitcher has never died, but fans and base coaches have.
having played third base (hot corner) at a fairly high high school level, I can say that you don't really think about it much in the field. but when you are hitting and the pitcher throws hard and wild, it's just about the most uncomfortable thing in the world

Possible, not not likely. They do wear a helmet, and a hit batsman is a walk, so a pitcher generally would never aim for it intentionally. Also, human reflex is pretty quick. They react fast enough to hit the ball you can generally get out of the way or protect yourself in time. Not always though. I don't think I have ever seen anyone hit directly in the face.

This one time my teammate completely busted the other teams pitchers kneecaps with the most amazing hit Ive ever seen.
We were about 13 so he made the homerun with the poor kid just lying there topkek
Not both his kneecaps obviously, but I remember hearing after the game it was shattered.
One day a pitcher will get a 120mph comebacker in the skull and die. Then we can look forward to the knee jerk rule making helmets mandatory for pitchers.
Another guy died around that time too diving into a wall iirc.
