1. Indirect freekick to defending team, possibly a yellow card, definitely a warning, picture does not really show the infraction.

3. Red cards, do not have to be shown and can do retroactively,
I would show a red card to the stands, and stop the player and send him back to the bench.

FA rules indicate that if a player who is to be subbed refuses to leave play continues. So I'd just resume play.

The picture does not show clearly show the player diliberatley kicking the ball at the blue shirted player, apart from a freekick, if you decided to hit the player with the ball in this manner you could get punished.
Even though you can get away with this to win corners, goalkicks or throwings, you are technically not allowed to do this.
