The book overall is more bleak and grounded than the anime. Arcs like the internet suicide cult and the pyramid scheme don't exist, for example. Yamazaki in the anime, in spite of his flaws, is given this "voice of reason" role, and assumes the moral high ground when dealing with Satou and Misaki. Those traits of his don't exist in the book, he's a lot of more mean, very neurotic, and him and Satou basically introduce each other into their respective vices (Satou gets him into drugs and Yamazaki gets him into loli and CP, believe it or not.) Watching the anime you can very easily call them friends, but the same can't really be said of their relationship in the book. It's more just two losers relying on each other.
Sempai is also somewhat more fucked in her original state. It's been a while so my memory may be off, but I'm pretty sure the scene where she wants to go to a love hotel with Satou is presented as a recent flashback, and at that point she is already married and expecting a child.
