The reason why so most rap is just variations of ghetto, gangsta hood shit is the same reason why every love song is essentially the same: it is just inherent to the genre. The vast majority of musicians' goals, especially if they are lesser known, isn't to be the innovator of the genre or present some grand magnum opus. It is just about putting your own flavor and spin on a genre that's pretty set in stone.

Rapping is a vocal style. The difference between rapping and singing is that they don't discretely pitch their musical intervals, but they're similar in that they're both in time. The difference between rapping and talking is that they are in time, but they're similar because they don't discretely pitch their intervals (into minor seconds, major seconds, and so on.). Another thing is that in Rap, there is a focus on the rhythmic, metrical, and syntactic arrangement of syllables and rhymes, the "flow", in order to create a certain rhythm/feeling/groove/rhyme scheme.
Rap is a vocal technique rather than a genre. And it's not "spoken", unless you think Gil Scott Heron was rapping. You probably think Konnakol is "spoken" too.
