It does really suck, i work in construction in finland so my boots are waterproof and steeltipped. I think the lack of oxygen during the day topped with sweat makes this happen, i tend to keep myself clean

The yellow stuff is dead skin, the skin is lose from the skinbed and is basically dead. You have been cutting it shorter and shorter I assume?
Go see a specialist and see if they can save the nail.
As I also see you are wearing steel tipped shoes you should get a bigger size, for steeltips are unable to mold around your foot like a normal shoe ( I have the same issue with my feet being wide, I get 1 size bigger for work boots ).
