We lose a lot of people. Something around the lines of 65% of cardiac arrests end in a "loss". So to answer your question on the first one that could have actually made it and we lost due to actual fuck ups was an 18 month old on the turnpike.

There was a passenger car vs semi truck accident on the turnpike that was witnessed by another ambulance. They grabbed the kid and put them in their ambulance and did abso-fucking-lutely nothing to help them. Once we got on scene and took the kid their heart rate was below 60 so we had to start compressions. Intubated, IV, and actual adequate mechanical respiration's brought the heart rate to around 120 which is normal for a child that age. We were about 30 minutes from the closest trauma center and I just sat there watching this kids heart rate plummet... as well as the BP. The kid went straight into the "red room" which is trauma surgical. After about 30 minutes the child coded with an autopsy report of blunt force trauma.

First person I "saved" would be a gentleman that was stung by a bee but never told anyone. He presented as a diabetic. Then went unresponsive, then coded. After 20 minutes of CPR he finally came to and started fighting the Lucas 3 CPR device so we stopped it. His pressure was shit but he was actually awake. By the good grace of me trying to correct his pressure I gave push dose EPI. Which not only fixed his blood pressure but counteracted the bee sting. He wrote us a nice letter that I keep hanged up.

I had a college student apparently bone a chick so good she orgasmed. He thought it was a seizure and called 911. We had to explain the birds and the bees to 2 colleges students.

The first few months are pretty nerve racking. Every call is a new experience. After about a year you start to get used to the general gist of things. After seeing 10-20-30 people just fucked beyond means you get used to seeing it. I'd say probably a year until you just get numb to it.

Nothing really stays with me. I have a great support group, family, and friends. I see a therapist once a month and I don't have any of the PTSD tendencies that a lot of the older medics have. A recent study shows that EMS workers have a higher chance of suicide/PTSD than actual combat veterans and police. So I tried to get on top of it and I feel it has worked very well.

Worst part of my job is the bullshit calls. It's absolutely infuriating knowing that people call 911 for complete nonsense.

any time there is a wreck involving the ambulance with a patient on it we have to call a second unit. We are bound by the same hit and run laws. So we would stop, call a second unit, and stay on scene. Luckily all of our ambulances have dash cam so we have found 95% of accidents are the other drivers fault. And because there is dash cam. They usually get cited with a shitload of fines and lose their license.

You must live in a very rural area. Best way to deal with it is to work for somewhere not so rural. Suburb/City is the best bet.

It is definitely possible to go to college and be a paramedic. However, most paramedic program require around 40 hours a week for 13 months to complete. So while it is completely possible, it is not probable or advised.

Paramedic school isn't necessarily hard. The worst part of it is balancing your time.

Even with great support, I think you gotta have a special psyche to do your job. I know I couldn't do it. Shit would haunt me for sure. Thanks for doing what you do, user!

I've never heard of a bachelor program in paramedicine. The only thing I know of is an associate degree. Which will teach paramedicine fundamentals and core ed classes. If you want to go pre-med and want to get experience in the medical field. I'd say at the very least get your EMT. I've had numerous doctors tell me that their years as a paramedic helped them greatly in their clinical rotations.

Also, most PA program and Nurse Practitioner programs REQUIRE people to get their EMT or medical assistant.

We had a trauma Alpha come in, single vehicle MVC with an unrestrained 18 y/o male in the back. They hit a tree, the driver went through the windshield. He arrived in PEA, we did a cardiac window and there was a huge tamponade that filled his entire thoracic cavity. He died.

The 18 y/o in the back seat flew forward and was stopped and cushioned by a 16 year old female in the front seat. She was pronounced dead at my hospital. The 18 y/o is a tetraplegic and has tried killing himself multiple times by rolling into the road on his wheel chair. He’s a frequent flier at my hospital in psyche.

I think they caught & arrested 2 medics last year where I work for pocketing the leftovers. It happens & not everyone has integrity, so they weed them out, but believe it or not, the biggest recent bust was a huge healthcare sting that caught a bunch of nurses and a few medics on producing and sharing statutory & underage stuff.

That being said, healthcare workers are just regular people (with stronger stomachs) that step up to a higher standard, but any one of them can get caught up in anything anyone else can do. It says a lot about the others with the willpower to make the right choices despite getting spit on and degraded by their “customers” every day.

Firefighter here. They set a list of work for us each day to complete from maintenance of the engines, upkeep and repairs of the station, and constant training of “mock” rescue scenarios in full gear all while being interrupted by the tones going off for fire & EMS calls.

That being said, YES. We do try to knock it all out as early as possible during the day so we can be lazy for the rest of our shift & hope the times go off as little as possible haha

Pic is what I did to my ankle while high/stupid. Then landlord asked me to clean chin high garbage filling second floor balcony. I had a "job " taking out trash. Tried to explain the hospital said not to get it dirty. Sit with leg up. Take anti biotics.

Looked like a face. Destroyed my ability to walk easily. Something about rubbing trash on it seemed bad.

Sorry for such a bland answer. I'd rather give someone narcan on 100 overdoses than have to go and tell their mother/father and (usually young siblings) that they have died. There's a lot of animosity towards drug addicts.

Theres a reason why we dont have overdoses with patients older than 50. They never make it. Most or people my age with siblings/kids in the 5-10 range.

EMS has trainings, station chores, and general upkeep too. Dont make it seem you're so special lol.

I'm here to do a job. I do my job, and I do it very well. What makes me more annoyed than anything else is the armchair warriors thinking they actually know what the fuck they're talking about. I know just as many great millenials, out of my graduating glass, that are now doctors, CEOs, business owners, as I do boomers that are leeches on society.

Ha you’re not wrong. I’ve played both sides & found that EMS is steady work, while Fire kinda binges. Work your ass off then sleep your ass off - if you don’t get interrupted by a 911 call.

Where I currently work though, fire engines run all the EMS calls and wait for an ambulance to get there from across the county which is dumb in my opinion.

Mad respect to my EMS friends though. Enjoy the perks & don’t sell out to fire if that’s your passion. It definitely isn’t worth it if you want to be a paramedic.

Yeah man. Around 70 fire stations here & only maybe a dozen ambulance units. All fire & EMS units are ALS. It’s Florida, ya know? Old folks & snowbirds always need EMS, so they keep our volumes high on top of the normal population’s volume, hence why every unit needs to be ALS.

Oh hell yeah. It was super infected. It's better now. I was still using when that happened. Been clean for half a year. Only quit twice. This is my second go. But I feel done.

Eh, to a certain point... We are aloud to use nursing judgement, which we flex our autonomy "at least I do" all the time. I've refused to give narcs/benzos before from a stupid ass internal medicine doctor.. I've refused to give replacement K on a dialysis patient, refused to continue Primacore with no other pressor on board on a post op CABG, etc.. But, if one of my intensivists or ED physicians order a CIWA, I 100% trust them.

ABSOLUTELY. I've seen nurses straight up tell docs to go pound sand. We are all bound on the basic principle of do no harm. If you think a doc is going to kill a patient. You have every right to deny their order.

get your PHRN and join the ambo. I have a few people educating nurses on how the prehospital field works and it's not enough. We need nurses to willingly request an uncomfortable ride along on an ambo

When I first became a nurse, I kinda played with the idea of doing flight nursing... I've done a lot of critical care transport from facility to facility, but I'll have to read into the PHRN.
