afaik, cuba healthcare systems works because doctors can't sell their services for the amount they would like as is the case in other countries.
Doctors wages are fixed and played by the goverment, the service is free for all the people. The life expectancy is high because they have periodic checks so early detection and treatment of illnesses is easier.
I don't know about r&d and advenced treatments (for cancer and such) but I heard they have equipment and facilities, albeit those are seldom used by natives, more for tourists who would like to be treated there.

Communism could work as long as everyone opts into the system, and somehow stays satisfied with the state of affairs. But imposing it on others will cause rebellion.

Defending real life communist governments is kind of a trap, keep it in the realm of theory. But if you really want to, you can zero in on the achievements of the USSR (literacy, ended homelessness, that kind of thing). It's insanely easy to counter that with accurate examples of it being a shithole, though.
