Not op but have had the exact same thing done.
To answer your question, most of what is on his back is blood that's just been smeared from the main op and that's not actually from any huge clamps, the scratch on the top left should be unrelated to surgery unless it was a back street operation.
They do use things to keep your back open but it should not leave any marks or cuts.

As for the risk, there is a chance you just get fucked but surgery and become a veggie, your spine rejects the metal and it doesn't work, you have an infection some time down the line and they need go open you up and re do it essentially if it's under the fusion and so on but if your spine is like mine or ops it doesn't stop till you're a hunchback, so it's worth it. I'm 2 years gone and have zero pain and look normal so it's all good as long as you have a decent surgeon.

Had to have alot of tissue ect removed due to risk of infection from a skanky ass knife

not op but same surgery had my rods put in a few years ago. Recovery can be a bit of a bitch I had a pin hole leak at the bottom of my scar that didn't heal for a week or two after. Full recovery time can be up to 7 months in that time you will rely on other people like crazy, go from slow awkward shuffle to walking normally and probably feel like its never going to get better and that you neck back and hips are all connected by the same bone. Eventually you'll get better and mobility will come back pain and problems will vary from person to person personally i have a small pull when i move my head too quickly to one side but i cant feel anything down either side of my ribs. when i had my operation they told me to avoid contact sports (a bit obvious) and swimming for at least 1 year after the operation. depending on your age you could wear a brace that would allow you back to correct over time but this an be bulky and a bit uncomfortable. Ultimately im glad i took the operation.
