I'm a dentist in sw va. not far rural swva but there are plenty in those areas.
ext for a non patient will run you something like 200. more complicated can be more but if you see an oral surgeon sometimes medical insurance will pick it up. there are free clinics all over as well that generally see working uninsured or underinsured. there's a mom project yearly in wise in july where you can get ext's for free.
if you go to a walk in dr appt they'll give you an antibiotic that will get it under control for a while.
