Just look at what WWE has had characters do in the past and play it off as real for the people portraying just for it to he a work.

I think when he was younger he did have cancer, it went in to remission, and in his regular bloodwork to keep on top of it, there was a "blip". I think Vince seen this blip and seized an opportunity while Roman had his medication adjusted to fix the blip. I doubt he had actually had a full blown case of the cancer growing.
So, yes. I think it's a real situation that was "worked" but management in to looking like something much worse than it was, for the sole purpose of gaining sympathy.
