Not fake but it is weird that WWE is keeping an air tight lid on all details. it's possible it's exaggerated or something.
It's real but they monitored and exacerbated the circumstances surrounding it to take him out and drop him back into the product with appropriate timing.

Its a billion dollar company. And in the world of billion dollar business its super passable that the whole Roman/Cancer thing is a lie. There is just so much money involved.
And think of this, people have done a lot worse for a lot less. People kill each other over 5k. You think a billion dollar business that prides itself on lying about its product to begin with would find it unjust to have a character play out a role about having cancer?? Just look at what WWE has had characters do in the past and play it off as real for the people portraying just for it to he a work.
My opinion considering the pros and the cons. Yeah this Cancer thing is a work.
