The needle kept her alive but only so you could kill her. I believe the betrayal wasn't even necessary, just Gowry's fucked up cultist lens. Malenia hated those faggots for a reason. Still that would mean Millicent would eventually stop being Millicent and either bloom as her own Valkyrie through betrayal or become something else by getting back with Malenia.
