you can brick 80% of the quests accidentally by going somewhere before doing something else which the game doesn't guarantee you'll find, even if you explore everything.
hints aren't enough. you can get told something by the 10th hour about a late game area and you'll forget it by the 90th hour.
characters teleport to other places randomly and you can gave no idea about that without a guide or without searching the entire map 100 times a playthrough, because

can all be triggers and you have no clue.
it's easy to miss items and npcs when the world is too big for the sake of being big, not for the sake of being filled with meaningful content.

No. The Lands Between represents Midgard in Fromsoftware world tree canon. What happens after letting the world darken in Ds3 leads to Sekiro and bloodborne.

Leyndell bricks it at some point, but going to Altus Plateau itself is fine. I think it has to do with first-time intro dialogue bugging out in Redmane, so it only happens if you also miss Alexander at his first spot.

Ive played more than you. Leyndell is absolute number 1 fantasy city in any game in terms of atmosphere, visuals, level design, music and immersion.
