The fact that this game even exists is nothing short of a miracle.

I'm not into piss at all, but on-paper it seems like something that could easilly be made into a game feature involving time limit.
It gets that objective-subjective connection. In a monster horror game, the "objective" is avoiding an unstoppable enemy, creating apprehension. The "subjective" aspect is how scary the enemy may actually look.
With an omorashi game, the "objective" aspect is the time limit and the pressue of failing to make it, and the "subjective" part is seeing the character's need to pee.

And knowing the longer you take, the more difficult mechanically it'll be for the character to walk or complete tasks.
