Fear & Hunger Termina. Hasn't been released, but the demo is out, you can check it out. It's way more polished and easier than the previous title - Fear & Hunger. Playing first game isn't technically required, but you'll be missing out a lot of story, so I'd recommend playing the first one
To be honest, it's kind of both. The dev is from Finland, so you can bet your sweet ass he makes games that are as dark as their soul, so, for example, there's anal rape. But he makes it so the anal rape is part of the plot, so it's good.
