In Fallout 4 presentation he said something about "Every button, every blinking light - everything is hand-made". While it's true that there are many small unique details in Vault 111 and the Institute, every other location in game sucks. Same terminals everywhere. American terminal running American Rob-Co OS on Chinese submarine. Same piles of junk and clutter everywhere. Same pieces of paper everywhere. They literally have one model for pile of junk that they place everywhere.

I hate to say this but FO4 has the best map of all the Fallouts. Of course they're not gonna place millions of unique pieces of paper or small items throughout the game, that's just not realistic even for a triple A company with near-infinite budget.
