Once I get him down to one life, I get one turn before he hits me non-stop until I am dead. Requiem doesn't work, break doesn't stop it. Dodge didn't help. I have almost 1mil HP so it must be % based attacks. How can I survive his autism onslaught?

Do you have K***ht's Sword maxed out? Use its buff. It's completely ridiculous, it doubles or even quadruples your survivability, can't remember
Requiem doesn't work on some of the bosses, in particular DLC3 ones. Just use the aforementioned buff when you want to survive

That's how I did it. I am sure there are other option, but I never was good with nuances. I simply grinded Mabel challenge room for black souls and maxed defence and hp. Stat limit is something like 64k I think.

890k HP and 34k DEF, once he's down to his last life he attacks me over and over, it triggers a counter which brings him to 1HP and then he just keeps hitting me till I die

Wow, worked like a charm user, equipping the sword brought my DEF to 43k and the buff kept me alive, thank you. If you couldn't save before bosses in the chaos dungeon, this would have ended me at dungeon level 177. Can't wait to be out of here

You just gotta beat the Old King now, he appears somewhere between floor 160-200. Should be around 180 on average, depends on your RNG with the depth counter (it needs to hit 9) and boss appearance.

Don't use regular stat boosting buffs since she dispels those and does damage. Knight's Sword and Holy Banner work well, though. I think you should also have access to Phalanx, which also increases your defense.
If you've gone through DLC3, use Meat Wall before she does the big attack. It will take at least half of the blows.

How bizarre. You should be able to "one shot" her with those stats, even at difficulty 9. Switching to the King Knight's Sword right before she does her attack doubles your defense, and its special skill doubles your HP (not that you need that at this point). I personally prefer wearing a lot of Black Rabbit Rings just for the extra turns. The increase in FP doesn't mean much when you have such a large mana pool.

Once the boss starts glowing with the book icon above the HP bar, the next attack it does is a 1shot. On phase 2 I can't get it below 1HP and it's mirror selves can all do the 1-shot as well. Requiem can't save me here, and I can't break 3-4 in one turn

I did attack the clones, they all have the same HP as the boss did at the beginning, and I can't one shot them. They must have about 1.5 mil HP if I had to guess
