That needle was just glued together with spit and Rot by Gowry. It was meant to just hold her together long enough to form a bond with (You), so when you killed her the betrayal would make her bloom.

Gowry's version however was meddled with and was instead transforming her into something else, probably trying to make her become an avatar of the god of rot like Malenia. That's why she decides to go out on her terms.

To drive the point home even harder they made it so her corpse never disappears. I literally just beat Radagon and went back to that spot, and she's still there.

There's literally no sign of her changing into anything else during the entirety of her quest, and needle seemed to have done it's job of simply keeping the rot at bay. Not curing it, but halting it.

You can talk to Gowry several times after Millicent departs, and you get special dialogue for buying that one REALLY suspicious rot spell he has. He's genuinely just knowledgeable about the rot and its literal children on account of being a cultist, and thinks Millicent turning into a Valkyrie is the best for both of them since he truly believes she's going to die to her sisters. It's solely the emotional trauma of betrayal that's supposed to trigger her awakening into a new persona or form, and I'd honestly take Gowry's words at face value because of how honest he is with you about /everything/.

On the topic of the needle, I'm pretty sure it's a point of irony that Malenia's death flower is the final thing needed to perfect the very thing meant to save her. She was doomed, if only from inaction, while Millicent chose to go out on her own terms if she was damned to die or lose herself like her sisters once more.

It's a great quest line because you can sympathize with Gowry too and his feelings of abandonment in a fucked up nightmare world where his only faith lies in the cycle of life and death rot brings with it. It's clear that he genuinely did love her like a father.
