Was in the same boat as you. Trying to maneuver the arena with all the blood wasn't working. Just get his first phase down as close to perfect as you can to preserve flasks. Ditch a shield if you've been using it and 2hand. Once the second phase starts just stay on him and don't worry about dodging the fire. Try to stay behind him and wail on him. You want to heal constantly. Assuming 40+ vig, heal as soon as you're around 2/3 health.

On Sulivus's quest you get two dolls, one is the lost friend of Alexander the warrior Jar and the other is a holy Maiden fated never to meet her tarnished. Melina also said in her introduction that you are a tarnished, fated to never meet their Maiden. I just took that to mean that Sulivus gave your Maiden a soul killing potion and turned her into a doll.
