You don't get it at all. The guy on cover is former pilot of experimental aircraft that retired after turning the tide of interstellar war against alien invasion. Government thinks it was destroyed in final battle but in fact he hid it in his barn since he ended up attached to it. So, in long, cold nights, he sometimes pull it outside barn, look at the skies and play his guitar, recalling all the memories from his prime years.

It's a bad box art, but it's marketing genius. Think about it.
It's October of 1992. There's already dozens upon dozens of space shooters on the market across consoles, arcades, and even PC and a good chunk of those come from recognizable series/companies. You're tasked with selling a game no-one has ever heard of from a Japanese-y-named company no-one has ever heard of (the Japanese-y bit is VERY important as the US was still in the grip of anti-Japanese-ism hysteria idiocy thanks to Japan starting to invest in America), with a Japanese cover that, while great """Japanese""" art, is so generic you can't do anything with it (and so phallic as to cause issues, the least of which are the obvious "phallic phalanx" jokes) and also so Japanese stores refuse to stock it as they believe it won't sell (Mega Man didn't get a Japanese art cover in America until 8 for this very reason, recall).
What are you gonna do to differentiate this game from all the other space shooters on the market & make people want to touch it just to find-out wtf it is, while also making it palatable for stores to stock?

The artist was interviewed by EGM I think some 15 or so years ago. Apparently he was only allowed to read the game description used on the back of the box & see only a few slightly blurry screenshots of the game for reference. He never saw any of the original art & never realized that Mega Man's eyes wasn't a face plate.
Mega Man got shit on so much. We won't even discuss his Captain N green version because the art guy had the hue on his TV wrong.

But also keep this in mind: The Japanese game released 4 months before the US version. Kemco didn't have an in-house marketing department; they outsourced for it. An outsource who had neither the time nor the artistic talent to Americanize the Japanese cover in a way that would catch peoples' attention.
And Kemco didn't care as they didn't believe it would sell anyway AND held veto power of the cover art besides. The fact they didn't veto that cover tells us everything.

Back in the 80's and through to the mid/late 90's, some magazine (Newsweek or Time, maybe? I honestly cannot remember) ran a story about how Japanese companies were investing heavily in America and buying-up formerly American companies. SOMEHOW this translated into "zOMG JAPAN INVADING AMERICA!!!", and suddenly stores refused to sell shit if it "looked too Japanese" out of a (legitimate) belief that it wouldn't sell (because they didn't), and stations refused to air programming without it being altered (hence, "Rock the Dragon"). But, so long as it didn't LOOK "Japanese", stores would sell it, and racist parents wouldn't realize it.
