Fetishes makes it easier for your character to become horny (which gives you a +2 str buff but -2 int debuff). You can jerk off to remove it, which also grants you a hp regen buff. But having sex with someone else who is way better than you also gives you more dopamine on release, which increases your hp regen buff. You can also make sexual interactions with you free for all, allowing anyone to just grope you and shit. There's also varying dicksizes depending on stats and shit. It's almost like a simulation.

Additionally, on things like facefucking players can't just stop it from happening (except attempt to chomp their dick off which 99% never works or you end up suffocating on the oversized dick). You can also get trapped by others players in their apartments with no way of recovery, other than finding something to kill yourself with, because logging out doesn't work. You can log out, but your character is always online.

Probably both. What would probably end up happening is that the team healers are also enemy capture-the-flag targets, and if you capture the enemy healer you can use them as a disposable weapon/shield/buff instead of wasting your own resources. It'd be a good way of letting healers be very powerful while keeping them dangerous to play.
