do NPC's permanently disappear if you die too much? Do you get permanently locked out of questlines or is there a way to reverse it?

Yes, they die, but you-ll be warned before it happens.

You will get an item called rot essence and if you die like 20 or 30 times after having that item, the NPC will die.

You can cure everyone inflicted with dragonrot with an item. When you get a rot essence, talk to emma and she-ll ask you to collect blood samples. Get a sample from the old lady in the ruined village at the start of the game, then go back to emma and she-ll give you the cure. it becomes a non issue.

The cure makes the disease go away, but they can catch it again if you die a lot.

Only characters you have met will be afflicted. As stated, the rot essence item will indicate who is afflicted. There is a rot essence for every NPC that can be afflicted by the disease.

What you wanna do is start a brand new game every time you die. It's the only way to get the very best true ending.

But the purification is not the best ending. The best ending is the return.

Also, user, this game has pretty old school requirements to get anything above the bad endings, that is, it has bullshit requirements that you'd never be able to guess by yourself, so you better read a guide if you're interested in getting the best endings.

You should do the purification questline up until you unlock the revisited hirata estate, to get that area of the game and get all the prayer beads, but DO NOT complete the area or you'll be locked from the return ending

Just keep pushing forward, soo. you'll have so many dragon blood droplets it'll become a non issue.

If you're really stuck on a boss just don't use the blood droplet until after you get past it, otherwise it might go to waste if you just keep dying.

Dragonrot is just the result of terribly explained mechanics.
Dragonrot does not kill NPCs, it simply halts progress for their quest. Many people thought that Dragonrot kills because...well the game explicitly says it and because the first character that can be afflicted with dragonrot, the old lady, DIES at the end of her questline, so people simply assumed that she died because the player got their ass kicked too many times by lady butterfly

They never really left, they simply had a publishing deal for Sekiro. If im not mistaken BN is still in the credits for Sekiro.

You can miss dialog and a boss towards the end of the game if you are not careful. There is no way to reverse it.
It's Owl 2, make sure you do the quest before defeating divine dragon.
