Don't get hit and don't take forever. Also find the optional fights. Combo really shoudln't be an issue most of the time if you don't get hit and keep attacking.

Don't use items or die
It's simple, once you know. The idea is to sustain your combo, avoid damage and do it as fast as possible. The last two come later but for now focus on maintaining your combo.
Those numbers on the top right are your combo meter. If you take too long dealing damage to the enemies it drops. To sustain it keep attacking. You can hold the attack button to shoot with your guns as you punch or kick. If the enemy is too far away you can use the regular shot to maintain your combo or, if they block, taunt to keep the meter from dropping. Start there and with practice you'll get them double P's.

Fairness and Fearless in particular can suck a fat dick, jesus christ, I hate those fuckers so much.
Keep your combo up with the generic pistol shot, maximize your dodge-offset efficiency.

If you're really going for it, if you so much as get hit, return to the title screen and then continue from where you left off. Thank god they mitigated this in MGR with a restart from last checkpoint in the pause menu.

If dieing didn't matter you wouldn't worry so much about getting hit.

The white/gold version are Gracious and Glorious, first show up in chapter 10. You don't get WT off of any of their attacks.
Maybe you're thinking of Grace and Glory. Gracious and Glorious are updated versions of Grace and Glory that show up in Non-Stop Infinite Climax and Hard, they can be a really big pain to deal with.

I can barely get through it on normal. Hard must be a fucking nightmare.
I don't do that bad. If I am hit once it's gold, twice it's silver, mostly I get Platinum or Pure. But I die, over and over until I figure out a near perfect run. I wish I didn't punish my score for when I keep trying.

In terms of W101 they're really slow about giving you your full moveset. You really are gimped until Operation 005.
In my defense I didn't realize Infinite Climxax still had Witch time. So I had the bomb brancer eqiped and had to defeat the Luman Sage with no Witch time.
Now it would be easy because back then I didn't realize how over powered the bird's spiral swords are. But at the time I had to defeat him by only damaging him with the bombs that drop when I dodge because he would dodge literally every single attack.

I would never go for PPPPP combos because I think they're too slow and enraged enemies can block you out of your WW, but I will intentionally fish for them when playing Jeanne just for that voice clip.

No, it implies that you missed one of the fatal QTEs that happen every second cutscene.
It's literally a non-factor if you pay attention and/or on any run after your first.
