I like that SH actually makes enemies more aggressive and difficult to deal with rather than just adjusting health/damage. The Slurpers in SH3 on Extreme+ always fuck up my 10 star runs.

Mount the ISOs of 2 & 3, and then run their installers like always.
DON'T use W10's own virtual disc app! Don't install under C:\Program Files\ !
The SH1 is a PS1 rip that you gotta emulate.
Follow the SH2 widescreen fix site's instructions to the teeth, and you should be fine.

SH's difficulties actually do all of those.
You need to pummel the grey kids & straitjackets a lot more on Hard than you do on lower modes.
High difficulties also add more enemies in many of the rooms.
Native Americans knew what the area had going on with it according to the history society in 2 and I think 4 even touches on it some more. It still doesn't give much of what they did about it or even a timeline until the settlers came in.

I played SH on hard for a bit one time. I ran into a door and got knocked back. I was shocked. Never saw that before.
if I recall correctly, that's a feature on all difficulties in SH1, but only becomes a thing on higher difficulties in SH2 and 3.
The sequels also add other little "realism" things as you increase the difficulty, such as your melee weapons actually colliding and ricocheting off the world's surface and enemies as you swing 'em, and those bottomless pits becoming actual death traps ya can fall into.
