Pressing the AZ-5 button causes all of hte control rods to insert at once as designed but the rods have a graphite tip which when it makes contact with water will vaporize it leading to a sudden increase in energy which in some circumstances can lead to an explosion

No. The rods are not graphite tipped. They are graphite halfway up and the rest is something else which absorbs neutrons. You pull them down or up to control ratio of graphite to absorber in the core to control neutron flux. Problem is when the rods are all up and you plunge them down in a SCRAM, the graphite displaces neutron absorbing light water and causes a local spike in neutron flux at the bottom of the reactor just before the rods can get deep enough for the other absorbing material to stop the reaction.
Simply put they were outside of their operating manual by getting into a situation where they had to pull out all the rods to over come poison build up.
