the lake with the island was there from the beginning. It was actually part of the second quest that was supposed to be located there, along with the Desert Quest, but the person making it (don't remember the name, but it was someone from outside of cipsoft team) didn't have enough time to finish it or something. The quest was supposed to use all those weird switch tiles under the desert/fire devil area to change the flow of the underground water tunnels (something that was hinted in Carlin - the only city at the time, that had canalisation system) draining the water from the lake and allowing you to get the items from the body. The items are there to this day, but unfortunately without the way to get to them.

it's much better than few years ago, because there's much much less botting and the game is much easier nowadays - you don't lose so much when dying and getting experience back is 10x faster than before. also because the game is more than 20 years old the amount of things to explore is astonishing.

backtracking, losing money or items when you die and pattern memorizing are not gameplay mechanics that make the game hard, theyre gameplay mechanics that make the game cheap

Not exactly difficult to finish a quest at any given time. The truth is that large chunks of the game were left intentionally "unfinished" so that players would continue infinitely searching for answers which kept them paying that monthly fee.
Have to hand it to CIP for finding a cheap way to hook retards into continuing to send them money on monthly basis.
