Maybe not rarest, but luckiest I ever had was getting a Liverto weapon at level 17 in Black Desert, back when they were considered second best to best in slot for many classes. It was my first day playing, and a friend was doing Red Nose scrolls when the real one spawned and high levels started piling in.
I didn't really stand a chance and died several times, but the spawn point was nearby and I figured out, as a warrior, I could shield some of his big swings and hit him at a reasonable range with the cone aoe sword slam and did that until he died. When he died, I hurriedly looted without really noticing what I picked up. I was more excited about the sizeable silver gain until my buddy noticed the local chat was going apeshit over the broadcast drop message. Had no idea it was like a once a day kind of thing.

It was the first time I'd ever played it since release and my friend had done the same with farming statues, he'd had the last prism and all this super op hard to grind loot, but never had a slime staff :p I was so chuffed dude then me and another new player found him one in his first few days of play!

Best rune I ever found was an Ohm. It was during a full Baal run too. It even sat on the floor for a good couple of seconds before I snagged it. Maybe they were all bots because no one said a word about it.

This in RS3 a few years back. It's a 1/16,000 drop rate from clue scrolls and sold for gold cap back in the day. I could have gotten way more for it apparently, but I actually sold it for 700m or so only to find out the GE gave me 2.4b instead. Needless to say I was completely fucking baffled and thought the money was a mistake - both at the estimated price when I looted it, and what I actually got for it. The highest amount of gold I had ever before that was like 30m or so.

Needless to say, I used that money to fund my first few 99's including Herblore and Prayer - Herblore namely being the skill I thought I'd get to 99 last due to the absolute insane money requirement to do so. I also bought a shadow dragon pet with it because I'd never spend real money on the game.

I still have like 1b left and use it to sub and play the game once a year or so to do all the new quests.

it's not necessarily that any borderlands 2 item is super rare, just that the drop rates can be really low combined with the fact that stats are randomized. it can take a long ass time to get the right one.

Grim Dawn has some super rare loot drops.

Aside from the obvious Legendaries the Prefix - Suffix system it uses on Rare items can generate some ridiulously unlikely items. Combined with the actual Monster Infrequents it's taken to a whole new level.

Yeah that's up there, maybe some of the untradables or tradable rares in rs3. Things like the max infernal cape can still be obtained and will eventually be slightly more common.

Regardless it was given purely because of an item duplication glitch that created copies of the "Spoppy" petpet with different IDs through the cooking pot.

Not the most rare item I've ever found in a game but this was the first thing that came to mind. A bit of a status symbol back in the beginning.
