This is a long-time part of how the tethers work. Since movement for individuals is client-side before updating the server, it's possible if two people have bad ping (or just one does) if they're both moving the server can accidentally skip a beat so the tether isn't grabbed. It's because the two characters are actually in different locations for each of their clients and for the server. You'll see yourself in one spot, but the other person might see you in another, and so might the server, because of how it updates movement.

This is normally only an issue with tethers, and the way to correct it is to have the person with the tether stand still. That's how it's always worked. If you're standing still they'll be able to grab it every time unless their ping is REALLY bad (in which case they have bigger problems) or they're jumping (which causes issues with the server-side update on movement).
