I'm guessing because its unproven for jap developers. Its pretty expensive to properly localize and ensure legal compliance.

Also I'm guessing theres a hardcore fanbase in japan that doesn't want their niche porn games being influenced by western tastes.

Koikatsu already had to cut the entire school mode from their Steam release to get published, which was thankfully unlockable for free online.

COM3D is interesting since it doesn't have the cross compatability with the japanese content, and you have to purchase the lewd patch externally. I'm surprised it has been selling. But I found the jap version configuration a PITA so others probably felt the same way.
