This reminded me that I hate the Necromancer in Diablo III because every single corpse was an identical mutilated torso. That was really the wrong class to have to add to the game as an afterthought.

NPC dialogue doesn't change everytime story progresses a little or I've been gone for a while

NPCs aren't capable of abstract thought. They see a TV and think "It's just a TV, what's happening on it is just lights." Just like a dog sees a TV. A person who can think abstractly can allow themselves to "position" themselves within the world that's being shown on the screen and think to themselves as if they were really there even though they know they're not.
