Something truly great will have both good gameplay and no ludonarrative dissonance, but ludonarrative dissonance doesn't matter at all. The narrative part is just set dressing for the fun. If something that minor automatically makes it a shitty product, then there are no good products because everything has minor flaws like that. The only way gameplay can be tainted by the story is if the gameplay is worsened for the sake of being more coherent with the story.

This only happens in linear games where the the player character is set up to have a preset story and motivations. Maybe the hack writers shouldn't do that or maybe but in restrictions to prevent the player from going off the beaten path.

The way reviewers use that term is just as a way to indicate when the gameplay doesn't jive with what's being presented in the story. Like in Tomb Raider 2013 when Lara is portrayed as this poor scared girl who's just trying to survive when you're gunning fuckers down left and right the entire game. Other instances would be when you obviously beat an encounter in a game, but the game shows you losing for narrative purposes, or when a game shows you doing awesome shit in a cutscene that your character can't do at all in actual gameplay.

I do like the fact that there's a break between gameplay and story. IF there was a strict adherence to following the plot in gameplay, then the game wouldn't be a sandbox, which is what Rockstar likes to make, and they're pretty good at that.

Ludonarrative dissonance is when you're playing a stealth game and there's a cutscene during which your main character gets discovered and the enemies raise alarms. Your character does something in a cutscene that you would never let them do during gameplay. Or a character who always hits a target and kills everything in one hit will miss multiple times in a cutscene. Or when cops can chase you forever ingame and there's no way to hide from them but your character is perfectly capable of evading them in a cutscene. Ludonarrative dissonance is when a game's story breaks the basic rules of the game in order to force a plot point that is basically incompatible with the game.

No it's the other way around. Ludonarrative dissonance harms gameplay for the sake of story. For example, Metal Gear Solid is the worst stealth game ever made because Snake gets caught in cutscenes and then has to fight the people that have caught him. Because the story demands that there's a fight. The player is explicitly meant to avoid confrontation in gameplay, but the story just disagrees with the basic gameplay.

I think you misunderstand what "narrative" implies.
Pacman has a narrative. In Pacman you eat pellets and avoid ghosts that want to get you, that's the narrative. If in between the stages you saw cutscenes of Pacman trying to solve world hunger with ghost scientists, that would be dissonant.
