Talk to her to find out it was actually Young Vlad that ordered the lockdown, and that he's planning on turning himself in to the Kin. Turn around and go back to Big Vlad and tattle on his son, he sends out goons to keep him from going. Go back to the Kin, tell them they ain't getting squat, they'll be mad but they won't do anything about it. They'll tell you to kill their dissenters. Tell them to fuck off. Go to Oyun and tell him you're not killing anyone. Proceed as normal until you do the Abattoir quest, jump into the pit, and see the Heart. When you go back, talk to the Kin again and now that you know the big Truth, they'll acknowledge you, no blood, no mess, no deaths.
How? He was in this pit with the Kin and he told me to make some diversion, I don't know how since I was busy killing the rogue odonghs

There are some pretty major differences in the story. Characters are quite different, entire plotlines new or distinct, new themes and setpieces. The broadest story is still largely the same: there still is the plague, the city, the centtal dillema, but A LOT of the actual elements and finer details, including the absolutely vital characters, are very much changed from the ground up.
I'm sure they'll blame tinybuild for doing such a shit job at marketing the game or fags like Forsen shitting on it rather then him.

Since those two (far from the only) sources of inspiration were released after the original Pathologic, Pathologic 2 is anything but "what Pathologic was initially meant to be".
Since those two sources of inspiration have essentially nothing to do with each other, Pathologic 2 has jack shit of integrity and internal consistency past the most superficial level.

The game already had a cult-like following. HD remake sold well over 100k in weeks after release. What is known is "that game that is probably amazing except it's inaccessible as hell". Publisher had no way to market it, it's an indie publisher. Look up how that fucking works. And user reviews were, for the most part, glaring.
The only things that went wrong pre-release was the idiotic naming decision and the fact that they underestimated just how dumb people are and did not write with MASSIVE GLOWING LETTERS that game contains only one episode. But judging from the amount of negative to positive user reviews, that hardly impacted anything.
The REAL, essential failure was, from start to finish, on the side of game jounalists. More specifically, that the quality of gaming journalism and review standards has dropped SEVERAL LEVELS since the HD game released. The fact that gaming journalists today are basically criminally incompetent, if not straight up malicious. That is all. That is what killed the game. And god knows how many other titles and talents are being destroyed every day due to this.
Gaming journalism has utterly and absolutely failed to preform, and has became one of the biggest detriments of the industry all together. And this game and its fate stands as a living proof.
So can anyone give me a quick rundown on how it relates to pathologic 1? I played the original but only the bachelor route so I was wondering if I should just continue there or if the remake is really just a retread.

It's pretty much a remake of the original, in this game you play as the Haruspex so you are in a good spot to continue playing on Pathologic 2
Fucking RPS dropped the ball in ways beyond the ken of mortal men. That review killed P2, I'm convinced. The game journos often work like a hivemind, if one big name place says it's good the rest often follow suit, and vice versa. If they'd only gotten someone competent without their head up their ass maybe the others would've seen it in a different light.
The follow-up reviews the RPS guy did were somehow even worse. An entire review about the train-summoning bit that seemed to exist purely because the writer got shamed by the comment section telling him (in relatively polite terms for once) he was a tasteless faggot, so he pads out this giant article about a cool but ultimately minor and meaningless event. And then the one after the difficulty slider patch release, in which he reveals AFTER TURNING DOWN ALL THE SLIDERS he still died so many times he took the fucking deal, and then got mad he didn't get a proper ending.

As it tends to be with IPL games, the relationship is kinda complicated, but to keep it simple:
It's functionally a remake of the second route from Pathologic 1 (Haruspex): keeping the same broad story beats and themes, but re-writing a lot of the finer details, and completely remaking the gameplay. There is virtually no reason to play Pathologic 1's Haruspex route now, other than out of interest in comparison, as Pathologic 2 retells it for the most part better: characters are improved, storylines have more depth (in most ways), and gameplay is actually kinda good this time around, actually enhancing the narrative experience instead of hindering it.
There is no need to play Pathologic 1 to enjoy the second, the only unquestionable advantage P1 has over the second one is the fact that it contains all three of the storylines.
I'd recommend playing Pathologic 2 now. And if you are really curious, you can go back and play Pathologic 1's Clara route, or even Haruspex later if you really want, but I doubt you'll will.

The funny thing is, if you go back and reread the review after playing the game, he got some pretty major plot points outright wrong, as well as missed some other really big parts of the story due to having only played Haruspex. It's still well written and conveys the feel of what you're getting into perfectly, so I'm very much willing to overlook that, but it's kind of funny how it's both iconic and, well, wrong.

Playing Bachelor's path will improve it a shit ton, I would say. His is a good introduction to the town and its themes and people without delving too deeply into the steppe lore and mysticism stuff, so that when you play 2 and they dump you directly in the deep end of all that, it adds that extra layer of recognition to the foreign-ness. Plus getting to see the different sides of all of the characters, and what they were willing to tell you as one protagonist versus what they were willing to tell the other. You *will* be lied to, and often.
To be fair I could see it happening because it took me a second to realise if the drawer or door on the thing your about to loot is dirty and grimy then it'll drop your immunity, and you might not realise this and try and loot the house of that guy you visit to get your fathers house back. The only areas infected are the spots were Anna and Peter live. There's also the death house with Khan and Notkin.
Not much, I think. I'd go for Pathologic 2 first, then go back to Pathologic 1 if you still feel like it. After all, you'd be replaying the (somewhat) same thing twice otherwise. P2 isn't exactly made with primary focus on veteran players, and I think exploring the town for the first time in it's P2-tier beauty is the best introduction to the games. There are some neat little nudges and nods to veteran P1 players, but not anything to justify subjecting yourself to P1's many, many problems.
Maybe it's due to the old translation? I heard Haruspex and Clara routes were abysmal.

It's a weird art game, for starters, which is already going to alienate a lot of normies who just wanna kill things and feel cool, but it's also punishing and difficult, which is a hard sell for huge swathes of the "Art" crowd, due to the increasingly common but deeply mistaken belief that all art should be accommodating, and pretty much every gaming journalist (even the few good ones) since they have to burn through so many games by such a deadline they have no patience for anything that doesn't immediately "Get to the good stuff," and if you're not prepared to accept the suffering as the good stuff then it's all downhill.
We're living in an industry where game journalists don't even finish the games they LIKE, where GB named the Witcher 3 one of their games of the year despite literally nobody on staff progressing past the Bloody Baron quest, which is like calling something your favorite book when you've read two chapters of ten.

Murky just has 1(one) sidequest worth of dialogue in the entire route about looking for her lost doll(which looks like a doll version fo the Haruspex)

The one I heard of was that they do a better job of building Haruspex and the Inquisitors relationship in Classic but they handle her knowing it's just a game bro better in 2.
The kids get almost zero development in 1, Murky especially. I think you talk to her all of once or twice. Sticky's barely there too. Lara, Rubin, and Grief aren't your childhood friends and none of that banter exists. Aspity is considerably more important and they play up her big crushie on Haruspex. The relationship with Haruspex and the Inquisitor is better fleshed out, but there's no option to skip the town and that entire part doesn't exist. The foreman (Oyun) straight up killed your dad instead. Khan gets more screentime and you actually get to visit him in the Polyhedron, but mostly you're just running errands for him.
The biggest difference is indeed:
1) Foreman is a cunt in P1 and you do indeed have a shitty bossfight with him.
2) Kin are FAR less developed in P1: they basically remain a completely inhuman mass of mute monsters. The whole theme of cultural conflict is virtually non-existent in P1.
3) The relationship with the kids is MUCH less developed. There is nothing that would compare to relationship you develop with Sticky and Murky. The kids are essentially distant means to an end to you, with one notable exception. You really do not get to "know" them too much.
4) The relationship between Artemy-Lara-Rubin-Grief does not exist at all.
5) Vlad is more of a treatcherous cunt, and will actively try to hinder you in P1, including throwing you into a prison at one point.
6) You do not really have the options to affect Rubins life as much.
7) Capella is the only one of the kids that gets more development in P1 than P2: she is far more friendly, and more involved in general.
8) The relationship between Algaya and Artemy feels somewhat smoother and more fleshed out in P1.
9) Bloch gets more screen time in P1 as well.
10) There is nothing that would compare to the Big Chamber moment in P1. The steppe lore is far more explained, and a lot less shown in P1 compared to P2.

Aspity, I'd say. But to be honest, the biggest problem with Haruspex in P1 is that he kinda does a whole lot of... nothing. Like entire days worth of basically waiting for the plot to move on. P1's Haruspex route was so much less involved than P2's one it's not even funny.
Later on, he hangs out a lot of with Algaya and a fair bit more with Victoria. And also, you have entire trial days which are just bullshit.
Sounds overall a lot more unfinished and bad to pathologic 2's Haruspex.

Since you mentioned them, how bad is changeling? I'm on day 10 and did Bachelor in classic, and thinking about going back and doing her route in classic at some point after.
The main missions for the day are typically "go to x did they start the plague" and report back. There are some interesting elements but the loop is even more repetitive than Pathologic already is.

First playthrough going, just met Lara for the first time and managed save myself from dying from exhaustion. I was attacked three times on the way to her house and barely survived. I was hungry and as soon as I got in her house I rummaged through cupboards and gulped down fish and bread like I was saved by Jesus Christ. As soon as I went to sleep and had a fucked up dream she wakes me up and tells me there's an intruder upstairs. Going up there with a kitchen knife as my only weapon and my health less than a fifth of the maximum was pretty intense.
So yeah. Is good game.

And no, Steppe lore is not explained better in the first game. Ignoring the fact that we are facing FIRST chapter of a three chapter story, and comparing it to second one, so it makes a lot more sense to hold back on some of the details, P1 dumped tons of exposition on you, where as P2 provided a lot of visual hints and generally woved the subject into the narrative more naturally. All of the subjects are brought up, they are just not exposed to death and the game lets you actually connect the dots a lot more.
As for your assessment of Algaya, you are hillariously wrong. In P2, it is actually explained WHY she knows what she knows. Where as in P1, it kinda feels like the developer got bored and just forced the meta-narrative into the game without much rhyme or reason.

P2: Yes.
P1: kinda.
In the first game, the survival mechanics are important, but they are rather shallow and mostly VERY easy to break. Most of the game is killing time, with just a small fraction dedicated to making sure you won't die.
In the second game, the survival mechanics are the heart of the experience, and very, very challenging and actually somewhat fun, and relevant to the narrative.
As for what happens:
IN P1, you need to complete the main quest on day 1, otherwise you'll get a game over. After that, as long as you stay alive, you can basically ignore everything, and the story will progress on it's own (albeit with largely undesirable consequences for you story-wise). If you ignore too many tasks, too many vital characters will die, and you may be eventually locked into bad ending.
P2 follows largely the same logic, except without the first day bullshit and some small neat twists that I don't want to spoil too much.
But in both games, the logic is that the story will develop even if you don't intervene: it's just that you'll have logical and natural reasons to try and have at least SOME control over how things are developing.

But the overall feeling of powerlessness is here. Unless you invoke the RNG gods there is no way you will save most people. Even staying alive is challenging since you have some kind of solitary worm that forces you to eat 5 fishes and three whole breads per day when you only have the money/barter to get a bottle a milk.

Yeah, that is a very, very important part of Pathologic, both the first one and the second one, although I would argue the second one does better job of it. In fact, the idea is that you kinda CAN'T do everything. And at times, doing nothing may in fact BE THE RIGHT CHOICE.
The massive, massive impact on the whole experience is that the narrative feels strangely far more involving and immersive than otherwise. The game makes you painfully aware that you are not the artifical pivot of the whole story, that you are just one of people trying to weather it. Another big difference between P1 and P2 is that each ending had their own implication and a player character standing behind it, Artemy for example was always for the steppe people and wanted to fuck up the Polyhedron, Bachelor protect it, etc.
