Real reason: Fate/Extra for PSP was the first game in the series that really was separate from the original VN. I guess they didn't feel secure in the game selling on its own or its series alone, so they included a number of characters which looked remarkably like their F/SN counterparts. You had "Saber" (who was a completely different Saber), "Archer" (who was, in a way, the same Archer), "Rin", and several others.
Fate/Extra Rin appeared to be the F/SN Rin in a virtual reality world, but it turns out that her real life appearance was different. She's a relative of Rin, with blonde hair, and so you got a new character who looks a lot like Rin but with blonde hair instead. Ereshkigal's design is clearly inspired to be Extra Rin, or at least to look like she is.
As for the in-game reason with Fate/Grand Order, that game has F/SN Rin (yes, the actual one) summoned into pre-history and becoming merged with the goddess Ishtar at the time. Thus, Ishtar looks identical to Rin. However, Ishtar is a two-faced goddess, and so when Ishtar was summoned, so was Ereshkigal. Since Ereshkigal was not summoned into a physical body, she has her full god powers, although she does look identical to her other half Ishtar. Also, she is a goddess and goddesses have blonde hair. (Except for all the ones that don't.)
So that's why Ereshkigal looks exactly like Rin, and why she has blonde hair. And why Extra Rin looks exactly like Rin and has blonde hair as well. And why the two are completely different.
