Have you ever encountered a no win scenario that forced you to reset the game and start over?
You can always level higher and then just build a separate character for pvp and co op levels. Both Dark Souls and Bloodborne are piss easy if you min max anyway.
I managed to have everyone survive except for one character in the final chapter simply because I had no desire to do both battles over.

Bethesda games almost require you to use at least 4 different manual save files at a time because bugs and shit.
When I was a kid I played Resi Code Veronica and didn't have ammo for the plane titan. Didn't know if that cargo chute thing was doing any damage to him. Pretty sure I had to restart that game a few times. Then I made it to Steve's transformation and I think I was too fucked to continue so I never finished it. Still thought it was a great game

Almost dropped the game during that sequence where you have to destroy the flesh hivemind or whatever the fuck that was. Almost had no ammo or healing.
Fallout 3. It was my first bethesda game and I knew nothing about it, so I got somewhere past Megaton without ever saving manually. I triggred an autosave and died instantly. I couldn't even live long enough to turn around and go back out the door. I've also done this with irresponsible save stating in emulators and not bothering to make real saves because it's slow.
That happened to me with system shock 2. It makes me wonder how designers avoid that. It never happened to me in Deus Ex and I've never spoken to anybody who had it happen either.
