This is my only experience as well. Never been banned from a game.

i got banned a couple of years ago for saying, and I quote, "lol xbox is bad"

I keep getting banned from a game about mice for making a spinning windmill

I still do this on ever server I own, but I put the reason as a generic racial epithet for blacks now since it's my server.

I always fly under the radar somehow. even doing stuff other people got b& for.
especially in MMOs. I was clipping through the ground to steal scrolls and jumping/gap closing through keeps in ESO and nothing.

No and it still pisses me off. In Gmod TTT back when it was big, there was one of the main servers everybody played on and I didn't have a mic back then. At one point I was an innocent with no weapon being chased by a terrorist, and desperately called out the terrorist until some detective and also admin runs out hearing the gunshots and starts chasing me and asking why am I shooting, I desperately drop everything and tell him that it's clearly not me who is shooting and he gets killed by the guy that has been shooting at me nonstop. The detective calls me out for RDM, the shooter doesn't want to give away his position and calls me out too, then other people jump in and they permanently banned me. The whole accusation part lasted a good half minute and nobody listed to me. Last time I checked a few years ago the server was still up.

Had the exact same experience except I killed a guy who was clearly bad and had made himself valid, but he was acting autistic and sperging hardcore like a child while i simply explained how I knew. It seemed to be some nepotist server and the guy must have been a friend of the admin or something, because the admin just went "lol ur retarded...." and permanently banned me after like 10 minutes of playtime.
The thing is, you're playing Gmod. That's the sole reason why this happens. Either you're an autistic kid playing it seriously or you're playing it just for a few laughs and to mess with those autistic kids.
