Majora's Mask is a direct sequel to Ocarina of time and she is at fault for the events that happen in Termina. Link leaving Hyrule is explicitly stated in the lore as to why the Hero of Time could never return to Hyrule. He "lost the qualities that made him a hero."

Zelda knew he would lose his Triforce of Courage, but she let him leave anyway. Making it another one of her consequential behaviors that Link had to rectify with the Ocarina of Time.

The ending to Ocarina of Time explicitly states Zelda would go back in time and meet Link again, having sealed Ganondorf in the Sacred Realm. Link would meet her, and he would leave. She knew everything she burdened him with.

The only reason the Triforce is missing in Twilight Princess is because of a literal plothole where Ganondorf gets the Triforce of Power anyway because laughinggoddesses.jog. Termina is just a parallel world that exist in the heads of Skull Kid and Link according to the 3 goddesses books, they treat it as an actual dimension that exist ala Lorule but it's said to be just a dream ala Kohorit island

The game sort of kind of explains it. The HMS was in Hyrule during the events of Ocarina but is implied to not be bound to a single world in his search for Masks. Termina's relationship to Hyrule is never explained but Granny's story rveals Skull Kid is from there which explains why it's his first target when he receives Majora's Mask. Since MM is defeated in Termina we never find out what risk it would have posed after destroying Termina.
