The fanmade ones are generally based on existing official adult patches or materials. These days it's pretty rare for fans to translate fresh material and make patches to implement it to official releases, mainly because most of the people who used to work on fan-translations and patches for visual novels now have official jobs relating to localization, and fan-patches are just something they don't have time for anymore.

I know but I do not wish to support a game without a official patch, it send the wrong message to the dev/publisher by telling them censorship is okay.
