In the highest difficulty, your max heatlh is 25 HP from the common 100 HP. Even if you get past the max heatlh you receive self-damage until you get back to 25 HP.
However, you have the Soul Cube with you from the beginning, which allows you to one hit some enemies after killing a certain amount of them and also gives you for a brief moment extra health.
The Soul Cube is a key element to the plot, so it is justified that you can only unlock Nightmare after you finish the game.
