Of the 3 endings, only one show you that you're out of the "nightmare" but you have to die to get the ending. So the ending could also be a dream. The other two, you rest in the world.
So there is more chance that you are in the real world during the events of the game.
Who knows, if it gets a sequel one day, we might know the truth

I'm just now playing it and even though its been thoroughly spoiled by Yea Forums for me I still love it as much as I did the first time I played Dark Souls

It is a dream, but dreams/nightmares here are taken from the Lovercraft mythos, meaning they are parallel universes existing alongside the real world. Both are real, and saying that "it's only a dream!" doesn't change anything since it still exists and still happened just on another plane of existence

I used to replay all the other games souls games quite often and always loved doing so, but Bloodborne was so perfect it completely ruined everything them for me and I can hardly stare at them without feeling disgust.
